Housing Stability Program Worker
$37.42 to $40.67 per hour
35 Weekly Hours
0830 – 1630
Reporting to the Supervisor, Hostel Programs, is responsible for assessing and verifying eligibility, delivering financial assistance and providing follow-up supports through a wraparound service delivery model to Regional clients on Ontario Works and the Ontario Disability Support Program (ODSP) as part of the Housing Stability Program (HSP), including referring residents to appropriate training, employment, health, financial literacy, housing, social services and community programs.
•Interviews applicants to assess and determine eligibility for assistance under the York Region’s Housing Stability Program Guidelines.
•Verifies information provided by the applicant to confirm eligibility.
•Determines available financial assistance and other potential income sources that may be available to applicants.
•Approves and issues benefits and authorizes disbursements and expenditures made to the clients or on behalf of the clients including 3rd party issuances.
•Provides case management through an initial assessment and through follow-up contacts with clients.
•Provides a wraparound service to support clients to promote housing stability.
•Develops a case plan and sets goals with clients issued HSP funds.
•Monitors client’s progress by contacting them at 2 weeks, 1 month, 3 month and 6 month intervals and make additional contacts as required to support housing stability.
•Maintains files and provides case administration in compliance with the appropriate Acts and Regulations.
•Complies with York Region Housing Stability Program Guidelines and documents recommendations for exceptions for Supervisor approval, as required.
•Utilizes social assistance delivery database, HSP electronic application and the Hostel Programs database to complete documentation, deliver services and manage caseloads.
•Records data for statistical tracking of program outcomes for provincial reporting.
•Seeks out Regional and external client supports.
•Responds to inquiries from internal and external customers regarding services and programs.
•Responds to concerns, as appropriate, from clients, service providers, community agencies and the general public.
•Provides information and/or presentations to individuals or groups.
•Performs other duties as assigned, in accordance with Branch and Department objectives.
•Successful completion of a Community College Diploma Program in Social Work, Social Science, or a related program.
•Minimum one to three (1-3) years’ experience in a social service environment, including experience with disadvantaged individuals, homelessness, mental health and senior population, employment counselling and group/workshop delivery and demonstrated skills to work in a demanding environment.
•Valid Ontario Class “G” Driver’s License and reliable vehicle for use on corporate business.
•Satisfactory Criminal Records Check.
•Knowledge and demonstrated ability in corporate core competencies including customer focus, communication, collaboration and personal ownership.
