Apotex has an opportunity for a


 (APPROX. 3-6 months)


OPSEU 595 - Hourly Pay Rate: $37.59 - $47.91 


The Apotex Centre, Jewish Home for the Aged is a 472-bed, long-term care facility which provides a range of residential and specialized programs. The small, home-like settings on each floor provide flexible schedules for programs, meals and personal care, based on each resident’s needs and preferences. Apotex residents require assistance to enable them to continue to live their lives and fulfill their goals and daily activities.



Responsibilities (include but are not limited to):

  • Provide tours of Apotex to individuals considering admission to Apotex and their families
  • Liaise with CCAC and the care team to facilitate admission to Apotex LTC
  • Provide individual, family and/or group psychosocial assessments, support and counseling
  • In collaboration with the inter-professional team, implement a client focused care plan and coordinate the discharge planning process for residents
  • Provide support/consultation to other disciplines on the team re: psychosocial issues particularly related to complex patient/family situations
  • Crisis intervention
  • Act as a linkage and communication conduit between the family and the care team
  • Establish community linkages and outreach with relevant agencies and other Baycrest programs and services
  • Participate in teaching and other student supervision activities for health professional students assigned to this unit


Qualifications (include but are not limited to):

  • Masters of Social Work degree
  • Current and valid registration with the Ontario College of Social Workers and Social Service Workers (‘OCSWSSW’)
  • Knowledge and understanding of ALC practices and principles required
  • Experience in discharge planning and system navigation preferred
  • Background in Minimum Data Set (‘MDS’) assessment and understanding of derived clinical outcomes preferred
  • Excellent interpersonal, communication and problem solving skills
  • Basic computer skills – Meditech, Microsoft Word and Excel, RMNR preferred
