Job Description

The Terraces Health Centre currently has an opportunity for a

 

REGISTERED NURSE COORDINATOR, FULL TIME

 

75 Hours Bi-weekly – Days/ potential to rotate (shifts may be subject to change)

 

ONA – Hourly Pay Rate: $37.76 - $49.95

 

The Terraces of Baycrest Retirement Residence provides older adults with the privacy of independent living, along with supportive services. It is connected to the Wagman Centre which provides a wide range of recreational, fitness, social and educational programs.

 

Responsibilities include but are not limited to:

  • Leads and coordinates (including staffing) the interdisciplinary team ensuring the provision of quality care that follows the guidelines of the Retirement Home Regulatory Authority (RHRA)
  • Leads the interdisciplinary team in a primary care model that empowers residents and promotes health and independence in community living
  • Implements, operates and evaluates new health promotion programs in both the Terraces and the Wagman Centre including professional clinics
  • Manages relationships with external vendors
  • Undertakes physical and psycho-social assessment of residents with complex medical, cognitive and social needs, and develops care plan
  • Provides support, education, and resident care including nursing to residents and families
  • Completes intake assessment of prospective residents, travel maybe required if assessment completed offsite
  • Liaises with Baycrest Hospital to ensure coordination of care for residents experiencing health difficulties
  • Liaises with community agencies and resources to coordinate services and establish partnerships
  • Liaises with professional practice chiefs
  • Assists with marketing initiatives
  • Provides support and education to the healthcare team members including RPNs, SW and other allied health
  • Assists in the clinical implementation of the new memory care floor as well as assisted living floors
  • Coordinates and runs health promotion/wellness nurse clinics for Terrace residents
  • Liaises with Terrace physicians
  • Provides leadership and support to quality improvement initiatives to ensure timelines are met

 

 

Qualifications include but are not limited to:

  • Current registration from the College of Nurses of Ontario
  • BScN required. Masters preferred
  • GNC(C) perferred
  • Current BCLS (CPR)
  • Minimum 3 years’ recent experience as a Registered Nurse in community health centres and/or retirement homes required
  • Minimum 1 year experience in a supervisory, clinical role required
  • Highly developed skills in clinical assessment of elderly client with complex medical, cognitive and social needs
  • Demonstrated ability to work effectively with and gain the trust of clients and families in difficult circumstances
  • Demonstrated ability to work collaboratively and effectively in the leadership of an interdisciplinary team
  • Excellent interpersonal and communication skills
  • Demonstrated problem solving and critical thinking skills
  • Excellent leadership skills
  • Proficiency in standard software packages (e.g. MS Office, email)
  • High-energy leader with strong conceptual and planning skills who has the ability to foster creativity, innovation and excellence
  • Superior interpersonal, communication and team-building skills, supported by a clinical background, are essential
