The Jewish Home for the Aged (Apotex) at Baycrest has an opportunity for a





75 Biweekly Hours - Days (shifts may be subject to change)


SEIU Service Hourly Pay Rate: $29.7050- $30.1857


Baycrest provides care and service to approximately 2,000 people a day through a unique continuum of care system, ranging from wellness programs, residential housing and outpatient clinics, to a 472-bed nursing home, and a 300-bed complex continuing care facility with an acute care unit. Baycrest is a world-class leader in innovation in aging, with the acclaimed Rotman Research Institute, considered one of the premier brain institutes in the world, and the Kunin-Lunenfeld Applied and Evaluative Research Unit (‘KLAERU’), which supports clinical research and application of research findings directly to patient care. Most recently, Baycrest was called to lead the The Centre for Aging and Brain Health Innovation (C-ABHI):  C-ABHI is a solution accelerator for the aging and brain health sector.  


Responsibilities include but are not limited to:

  • Manages the coordination ad completion of the RAI MDS program components
  • Ensures all the RAI MDS 2.0 documentation is completed by the deadline and reports any conflicts to the Director of Care and Resident Experience or designate
  • Ensures all the RAI MDS 2.0 documentation is completed by the deadline and reports any conflicts to the Director of Care and Resident Experience or designate
  • Ensures the RAI MDS 2.0 program is aligned with the MOHLTC program standards and organization policies to sustain all necessary documentation of resident care
  • Contributes to the development of RAI MDS policies as needed
  • Works in consultation with the Apotex Leadership Team and support services to ensure data integrity, data utilization and interpretation
  • Acts as the RAI MDS 2.0 resource for all participating team members in the home
  • Acts as the point person for team members in supporting them with their AIS related activities
  • Seeks professional development through attendance at seminars, workshops, in-service programs, reading literature pertinent to the profession and other skills development initiatives


Qualifications include but are not limited to:

  • Successful completion of a recognized Practical Nurse program
  • Current certificate of competence in good standing from the College of Nurses of Ontario.
  • Current Basic Cardiac Life Support (‘BCLS’), Level C.
  • Recognized First Aid certificate.
  • Current AIS Certification
  • MDS Education is an asset
  • Current and acceptable Vulnerable Sector Screen from the police services in the municipality of residence.
  • Must possess a minimum of 3 years clinical experience
  • Must possess a minimum of 2 years RAI MDS 2.0 related knowledge and practical experience
  • Previous experience in adult education/facilitation
  • Excellent verbal, written and listening communications skills.
  • Ability to document concisely and appropriately.
  • Demonstrated ability to effectively communicate and develop partnerships with clients and families
  • Good understanding of the relevant provisions of current legislation and regulations governing care, including: Long-Term Care Homes Act 2007 and Regulation 79/10 (Apotex), Ontario Public Hospitals Act 1990 and Regulations (Hospital), Residential Tenancies Act 2006 (Terraces), Personal Health Information Protection Act 2004, Freedom of Information and Protection of Privacy Act 1990
