Day Treatment/Integrated Community Care Team has an opportunity for an

 

PHYSIOTHERAPIST, TEMPORARY PART TIME (APPROX. 2 MONTHS)

 

60 Bi-Weekly hours / Days (shifts are subject to change)

 

OPSEU 583 Hourly Pay Rate: $33.59 - $46.56

 

Be part of Baycrest’s interdisciplinary specialized geriatric outpatient team. Working on both the Integrated Community Care Team (ICCT) and in the Day Treatment Centre (DTC) you will offer therapy to clients living independently in the community who can benefit from an interprofessional team to address issues that are having an impact on function. The Physiotherapist facilitates therapeutic group programming as well as working individually with clients to attain maximum physical and functional goals. Physical, cognitive and psychosocial issues are addressed. Your assessment and goal setting will inform the comprehensive geriatric assessment of clients. As operational needs require, the Physiotherapist may be re-assigned to other areas.

 

Responsibilities include but are not limited to:

  • Conducts client assessments, treatment planning and intervention for older adults in an outpatient hospital, home, or virtual care setting. Plays a key role in clinical decision-making and client advocacy for clients with dementia or other progressive neurodegenerative disorders and helping clients with independent living in the home
  • Provides comfort with innovative approaches to dementia care
  • Supports a systematic, evidence-based approach to client care, program development and evaluation, and professional practice
  • Participates actively in the teaching and mentoring of physiotherapy and physiotherapy assistant students
  • Presents cases and recommendations at team rounds
  • Assigns the patient plan of care, and provides direction, supervision & monitoring of physiotherapy/occupational therapy assistants
  • Prepares and delivers health-education to groups of patients
