Job Description

TBSU Transitional Behavioural Support Unit (TBSU) has an opportunity for a

 

SOCIAL WORKER (MSWI), PART TIME

 

42 Hours Bi-Weekly – Days (shifts may be subject to change)

 

OPSEU 595 – Hourly Pay Rate: $37.59 - $47.91

 

TBSU is a 27 bed unit which houses the 23 bed specialized transitional behavioural support unit as stipulated by the Long-Term Care Homes Act 2007. The residents on this unit receive a higher level of support that they would in other long term home environment with enhancements to both care delivery and physical environment.

 

Responsibilities include but are not limited to:

  • Provides tours of Apotex to individuals considering admission to Apotex and their families
  • Liaises with CCAC and the care team to facilitate admission to Apotex LTC
  • Provides individual, family and/or group psychosocial assessments, support and counseling
  • In collaboration with the inter-professional team, implements a client focused care plan and coordinates the discharge planning process for residents
  • Provides support/consultation to other disciplines on the team re: psychosocial issues particularly related to complex patient/family situations
  • Crisis intervention
  • Acts as a linkage and communication conduit between the family and the care team
  • Establishes community linkages and outreach with relevant agencies and other Baycrest programs and services
  • Participates in teaching and other student supervision activities for health professional students assigned to this unit

 

Qualifications include but are not limited to:

  • Masters of Social Work degree
  • Current and valid registration with the Ontario College of Social Workers and Social Service Workers (‘OCSWSSW’)
  • Knowledge and understanding of ALC practices and principles required
  • Experience in discharge planning and system navigation preferred
  • Background in Minimum Data Set (‘MDS’) assessment and understanding of derived clinical outcomes preferred
  • Excellent interpersonal, communication and problem solving skills
  • Basic computer skills – Meditech, Microsoft Word and Excel, RMNR preferred
