The Behavioural Support Community Outreach Team has an opportunity for a

 

CLINICIAN LEADER, TEMPORARY FULL TIME

(APPROX. 1 YEAR)

 

70 Biweekly hours - Days (shifts may be subject to change)

 

OPSEU 583 Hourly Pay Rate: $45.72 - $49.67

 

Baycrest has been appointed as the health service provider accountable for leading the implementation of the Toronto Central Local Health Integration Network’s Behaviour Support Action Plan. A key component of the plan is the deployment of a Community Behaviour Support Outreach Team (‘CBSOT’), consisting of three interdisciplinary community outreach clinicians who have behavioural support expertise. The Clinician will travel to various community locations in the Toronto Central LHIN and work with the inter-professional team to provide collaborative and evidence-based care.

 

Responsibilities include but are not limited to:

  • Provides comprehensive, clinical, in home assessment, with a focus on identifying factors contributing to responsive behaviors utilizing a bio-psychosocial framework.
  • Collaborates with client, care partners, and the client’s circle of care to develop and disseminate individualized behavior management plans.
  • Works side by side with Toronto CCAC teams and other community providers to provide behavioural support expertise, coaching and building capacity through partnerships.
  • Acts as system navigation support for clients with responsive behaviours along the care continuum to make referrals to other appropriate supports as required (e.g. Community Psychogeriatric Teams).
  • Collaborates and liaises with hospital, community groups, crisis teams, and other community teams to develop an integrated pathway for clients
  • Participates in data collection and reporting on a number of accountability measures.
  • Performs teaching responsibilities, participates in relevant education and research activities, and performs duties consistent with the job classification, as required.
  • Performing departmental responsibilities to contribute to the efficient and effective provision of service.
  • Ensures the protection of client rights and acting in an advocacy role for the resident/clients.

 

 

Qualifications include but are not limited to:

  • Completion of a Master’s degree program in a healthcare profession.
  • Current registration/licensure in good standing as required by a professional body.
  • Minimum three (3) years related experience in geriatrics, community based seniors care and behavioural management
  • Experience/familiarity with Resident Assessment Instrument—Home Care© (RAI-HC) and behavioural, cognitive, psychosocial, and environmental assessments relating to responsive behaviours
  • Demonstrated expertise and knowledge of working with individuals with various types of dementia, delirium, and those with acute behavioural change
  • Demonstrated competence in the use of standard psychogeriatric assessment tools and behavioural support management approaches
  • Demonstrated excellent clinical-reasoning, critical thinking and problem solving skills
  • Training in or knowledge of Gentle Persuasive Approach (GPA), P.I.E.C.E.S. U-First!, Montessori Approach
  • Effective assessment, intervention, client centered and collaborative care planning skills
  • Experience providing caregiver education, support and counselling relating to caring for a person with responsive behaviours
  • Demonstrated experience working collaboratively with primary care and specialists in the care of older adults with responsive behaviours
  • Ability and willingness to travel to travel in the Toronto LHIN area
  • Proficient computer skills (Microsoft Office Suite) and the use electronic communication devices and software pertaining to risk management and RAI documentation
  • Experience with program development, evaluation and quality improvement initiatives
